Accidente cerebro vascular isquémico como reactivación de lupus eritematoso sistémico

Presentamos el caso de un varón adulto con lupus eritematoso sistémico (LES) que ingresó al Servicio de Urgencias con historia de hemiparesia faciobraquial derecha y disartria. El ictus isquémico se confirma con los métodos auxiliares de diagnóstico. El perfil colagénico fue positivo para IF-ANA, anti-ADN e hipocomplementemia. Los anticuerpos antifosfolípidos fueron negativos. El tratamiento se realiza con corticosteroides y anticoagulación. Este caso pone de relieve la importancia de la detección precoz y el tratamiento oportuno de estas manifestaciones en pacientes con enfermedades del tejido conectivo como el LES.

We present the case of a male adult with systemic lupus erythematosus (SLE) admitted to the Urgency Service with a history of right faciobrachial hemiparesis and dysarthria. The ischemic stroke was confirmed by the auxiliary diagnostic methods. Collagen profile was positive for IF-ANA, anti-DNA and hypocomplementemia. Antiphospholipid antibodies were negative. Treatment was made with corticosteroids and anticoagulation. This case emphasizes the importance of early detection and opportune treatment of these manifestations in patients with connective tissues diseases like SLE.

Púrpura trombocitopénica en mujer adulta

Se presenta el caso de una mujer adulta mayor, previamente sana, que ingresa con antecedente de cuadro confusional, debilidad generalizada, cambio de coloración de piel y mucosas, síntomas gastrointestinales y fiebre. En estudios de imagen no se evidencian lesiones encefálicas. Ante cuadro clínico y hallazgos laboratoriales se realiza frotis de sangre periférica, confirmándose diagnóstico de púrpura trombocitopénica trombótica. Se indica plasmaféresis, sin embargo, la paciente no responde a la terapia y fallece.

This is the case of old adult woman, previously healthy, admitted with history of confusion symptoms, general weakness, change of skin and mucous color, gastrointestinal symptoms and fever. Medical imaging studies do not show evidences of encephalic lesions. Considering the clinical symptoms and laboratory findings, a smear of peripheral blood is made confirming a diagnosis of thrombotic thrombocytopenic purpura. Although plasmapheresis is indicated, the patient does not respond to the therapy and die.

Tumor fibroso solitario de pleura

Presentamos el caso de una mujer de 41 años, asmática, que ingresó al Servicio de Urgencias con una historia de dificultad respiratoria y tos, al examen se constata matidez del hemitórax derecho. Analítica en sangre dentro de los parámetros normales. En radiografía y tomografía de tórax se observa un gran tumor en hemitórax derecho de 20 por 20 cm con efecto de masa. La paciente fue sometida a toracotomía exploradora, realizándose extirpación completa del tumor, con buena evolución postoperatoria. Anatomía patológica confirma tumor fusocelular.

We present the case of a 41-year old asthmatic woman that was admitted to the Urgency Service with a history of respiratory difficulty and cough. At the clinical exam, dullness of the right hemithorax was found. Blood analytics was within normal parameters. Chest X-ray and thorax tomography showed a large 20 x 20 cm tumor in the right hemithorax with mass effect. The patient was subjected to exploratory thoracotomy with full extirpation of the tumor with a good post-operatory evolution. The pathologic anatomy confirmed a fusocellular tumour.
